Insurance Pricing
We are in network with BlueCross BlueShield, UnitedHealthcare, and Medicare. Everyone’s insurance plan is different; therefore, pricing may vary from plan to plan. If you would like to have our office check your insurance plan in order to provide you with an estimate of care, then you may reach out to the office directly by either e-mail, phone call, or text. You will need to be prepared to provide your insurance information along with your date of birth.
Self-Pay Pricing
Our self-pay pricing is listed in the table below. If you would like a definite estimate of care based off of what you are wanting to have treated in our office, then you may reach out to the office directly by either e-mail, phone call, or text.
Patients under 2 years of age are seen as standard new patients and are billed at the regular established patient visit rate. No regional add-on fees apply.
